- Click Quizzes, Surveys, or Self Assessments on the navbar.
- Click Question Library from the tool navigation.
Create surveys and use the statistics tools to monitor current course trends, seek opinions, and assess user satisfaction. Surveys are an excellent way to solicit feedback from participants regarding any aspect of a course. For example, use surveys as a method of collecting course evaluations, mid-year reviews, or researching people's learning styles and content delivery preferences.
Question types available to the Surveys tool are similar to the questions available in the Quizzes and Self Assessments tools. Surveys can contain the following question types:

TipAs a best practice, we recommend that you create all survey questions within the Question Library [37]. This enables you to reuse questions in various quizzes, surveys, and self assessments.
All survey question types allow you to include Feedback. Feedback enables you to provide added information as a basis for improving content comprehension after a user response.
Creating a survey involves a series of steps. After you name a survey, you can define its general properties, restrictions, objectives, and reports setup.
Category Group your survey under an existing category or create a new category. Categories are useful for organizing surveys with similar or related content.
Feedback Users receive feedback immediately after answering a question.
Anonymous Hides user data in survey results. The responses to survey questions are available for all users, but the system does not report who made what response.
Click Add/Edit Questions to create survey questions.
Click on a question from the Survey Layout section to preview it. Click Edit Values then select or clear the check box beside each question to modify their "Mandatory" answer status. Click Add/Edit Questions to create new questions, sections, information, and manage other question properties.
NoteYou can also edit values and preview questions in the Add/Edit Questions area.
Once you add questions to your survey, you can enter a value in the Questions per page field to indicate how many questions to display per page. Limiting the amount of questions available at a time is less overwhelming than placing an entire survey on one page. You can also choose to prevent users from moving backwards through pages once they have left a page of questions.
Create a description and submission message for your survey. Manage visibility options by selecting on or off.
Description Create a description at the start of the survey. Users will see the description at the top of the page before the first question.
Submission Message Display a message to users immediately after they submit the survey.
Add information or instructions to the bottom of the survey page. Change page footer visibility by selecting on or off.
Setting your survey status to Active allows users to see and take the survey. You can specify a date range that your survey becomes available to users. You can also add surveys associated with a date to your course calendar.
Create new, or attach existing release conditions if you want a survey to be available to users only after they fulfill other specified tasks. Examples of release conditions include completing a dropbox submission, scoring a specified grade in a particular quiz, completing a self assessment, and being enrolled in a particular section. You can edit and change release conditions by clicking the Remove icon or Remove All Conditions.
Set a response type to specify the number of survey attempts users are allowed.
Unlimited Users can answer and submit a survey as many times as they want.
Single attempt that is editable Allows users a single survey attempt. They can re-access and edit responses as long as the survey is still available.
Limited Sets a number of permissible survey attempts.
You can search and assign specific users a different set of survey availability dates. Special access properties enable you to increase the time limit for special-needs users and allow different start and end dates for individual users.
Add a rubric to your survey if you want to evaluate the overall quality of survey responses.
Associate a survey with learning objectives that you want users to master. See Associating surveys with learning objectives [13] for more information.
Use survey reports to gather information on survey data. Survey reports are different from survey results; they collect and present more types of information than is available through results. Choose a report type to display question statistics, question details, users statistics, attempt details, or user attempts. You can also select a release date and choose which roles have permission to view each report.
Create survey sections to organize your questions into folders. You can do this in the Question Library or while creating a survey. Both of these areas are identical in functionality: one creates folders within the Question Library, while the other creates folders within a survey.
You can also import sections from the Question Library directly into a quiz, survey, or self assessment. Importing a section transfers the section folder and any properties associated with the section (section name, messages, images, or feedback). You can still import all of the questions within a section separate from the section itself by creating the section within a quiz and clicking Add/Edit Questions.
NoteAlthough you can create subsections within sections, we recommend you keep question organization simple and intuitive.
NoteManaging survey sections (renaming, reordering, publishing, deleting) is similar to managing survey questions.
Manage multiple surveys at the same time. You can edit common properties such as survey name, category, and active status.
A copied survey contains all of the questions from the original survey. Copying is a quick and simple way to create a survey that assesses prior and current understanding of course materials.
NoteYou can only copy surveys within the same course.
The survey preview option allows you to test the accuracy of content before you release a survey. You can answer questions, submit survey, and preview the description and submission message
Change the order of surveys on the Reorder Surveys page. Surveys reorder within their respective categories if more than one category exists.
NoteTo reorder categories, see Creating and managing survey categories [11].
You can organize surveys into categories to make it easier to find assessments with similar or related content. Create, edit, reorder, and delete multiple categories at the same time.
Change the order of categories displaying on the Reorder Surveys page. The "Without Category" is always at the top of the list.
NoteYou can click Sort Order to sort categories in ascending or descending order.
If you have Multiple Choice and True or False questions in your survey, it is possible to add branching. Branching is an optional feature that a site administrator must enable for your course offering.
NoteWhen branching is used, survey questions are presented one at a time, each on a separate page (in other words, page breaks are inserted between every question).
Use the Objectives tab on a survey's Edit page to associate the survey with learning objectives. By associating surveys with learning objectives, you can tie assignments to competency structures and competencies you want users to master. Associations can be basic associations or associations with rubric-based assessments.

If you have permissions to edit a survey's associated learning objectives and assessments, click Edit Learning Objective or Edit Assessment from the context menu of the learning objective or assessment you want to modify.
The Question in Use page displays when you save changes to a shared question or shared information item. You can select the check box beside a quiz, self assessment, survey, and Question Library in which you want to apply your current changes and click Save.
The preview page allows you to view the appearance of an individual question, as well as the comments and hints you might include with the question. If the survey question appears in more than one place, the preview page also displays a list of places the question is in use (for example, Question Library, quizzes, surveys, self assessments).
Change the order of questions displayed in a survey by reordering them in the survey question list.
You can import survey questions from numerous sources. Questions can come from an existing collection of questions (quizzes, surveys, self assessments, and the Question Library), a CSV file, or a learning object repository.

You can create survey questions offline using a CSV formatted text file. Follow the steps below to access a template for creating the text file, or upload a question file you have completed.
NoteAssociating a creative commons license automatically populates the metadata rights fields with this information.
Note Based on how you have set up the survey properties, you might see a list of all users or just the overall survey results with anonymous responses.
You can export survey statistics and reports to a CSV file for data analysis. The CSV file contains all pertinent information from the survey so that you can query, sort, and evaluate the data without being connected to the learning management system. You must set up survey reports from the Reports Setup tab for the appropriate survey before you can view or export reports. See Creating surveys [4] to learn more about creating reports.
